COMPANION CARE SERVICES

SKILLED NURSING

Residents who require basic medical care, assistance with daily activities, or supervision will be assisted by a Companion if suggested by a medical professional. Limitations may apply as it will be predicated by the facility’s Director of Nursing. As a result of strict monitoring, extensive note-taking and written reports will be routine to document progress, or lack thereof, of each Resident.


ASSISTED LIVING

If you’re looking to maintain independence, but require assistance with daily activities, health monitoring, and personal care and safety, having a Companion is highly suggested.

Some personalized or suggested assistance could include:

ADLS

  • Bathing and personal hygiene

  • Dressing and grooming

  • Toileting and continence care

  • Mobility and transfer assistance

  • Eating/feeding assistance

Health & Medication

  • Medication reminders/administration

  • Monitoring general well-being

  • Documenting observations of changes in condition and reporting them to respective family and providers


INDEPENDENT LIVING

Typically, Residents who are looking for convenience, safety, and social benefits of a supportive community while maintaining complete independence will typically stay at an independent living facility. However, some assistance may be sought after to alleviate chores, stressors, or simply accommodate.

Household Support

  • Light housekeeping

  • Laundry service

  • Minor/basic repairs in apartment/cottage

  • Minor/basic landscaping

Dining & Hospitality

  • Meal cooking

  • Community dining assistance

  • Meal delivery assistance

Social, Recreational, & Wellness Programs

  • Organized social events/clubs

  • Fitness and wellness classes

  • Educational programs and hobby groups

  • Group outings, cultural events, or trips

Transportation

  • Shopping and errands

  • Medical appointments

IN-HOME CARE

This service is typically suggested to maintain a certain level of care upon discharge from a facility or hospital.

  • Dressing and grooming

  • Mobility and transfer assistance

  • Eating/feeding assistance

Emotional Support

  • Conversation and companionship
